I rehomed Maisie from her original owner. She was called Lola, which for various reasons I changed.
Her previous owner offered to keep her insurance running for a bit, an offer I accepted, and she changed the chip details.
I didn't tell her I'd changed Lola's name, and when Maisies booster was due, registered her at the vets as Maisie.
When I took the insurance over it didn't occur to me to change her name.

Some time down the line, Maisie injured her leg really badly. It was touch and go if she would have to lose it. It took months to get to the stage where I thought it safe to put in the insurance claim.

And then I realised - all the paperwork was in the name of Lola, but the vet bills were for Maisie.

I rang the co. They stated ( not unreasonably) that they would not pay out, unless I could get a letter from the vets confirming that Lola and Maisi were the same dog.

I'd weeded out a lot of paperwork and knew I couldn't ask the vet to confirm without some paperwork to show him.
I made up my mind that I'd lost the money (a very substantial amount)

Then, by chance, I looked at her vacc cert as I was putting it away . It was the original from the previous owners vet, and it had details of her chip on it.

I wrote to my vets, explaining the situation, and they agreed to try and help.

They scanned Maisie, whose chip obviously came up the same, and the vet recognised her as maisie ( pressure sore scars and all!) and they submitted my claim with a covering letter.

I am pleased to say that PetPlan met my claim in full
So
make sure the paperwork matches and let your vet know your dogs previous name for their files
keep all the relevant bits of paper
stay with the same vets
